“Organisations, even those truly focused on becoming ‘Learning Organisations’ fail to learn from their high-volume, low potential consequence events, rarely understanding the root causes. In general, the causes are not assessed using the same set of root causes as the high consequence incidents. Frankly speaking they do not match.
Root causes are not influenced by the seriousness of an event, therefore in terms of learning opportunities, improved understanding and trending of low consequence incidents offer greater capacity to learn than the less-frequent high consequents events.
COMET is the first Incident Management Process to solve this dilemma, by using a two-tier process that supports high consequence events (COMET) and low consequence events ) ‘COMET 60’). Critically, both apply identical root causes. The key difference is that COMET 60 requires significantly less time and resources and thus offers a practical solution to this dilemma.”
